Response to EU rules on medical devices and in vitro diagnostics - targeted evaluation

Within the Erasmus MC the Medical Technology Committee is responsible for the quality management system for safe development and application of medical technology within clinical research and clinical practice. We would like to address our concerns in line with the main concerns as presented by the European University Hospital Alliance (EUHA). EUHA is formed by premier European university hospitals with demonstrated excellence in healthcare, education and research. (see also their reaction on this evaluation): One of the main concerns of the Erasmus MC is the rising unavailability of medical devices. In practice we see that a lot of devices are taken from the market and in most cases without a timely announcement nor a suitable alternative available. This is especially an issue in University Hospitals that focus on highly specialized tertiary care where the use of very specific devices is necessary. We also see a risk of additional costs and a growing need for specific additional expertise. In times of growing demand on healthcare delivery, growing healthcare costs and a lack of (para)medical employees, to be able to comply to these regulations, additional expertise on product safety and product development (quality assurance, regulatory compliance, subject matter expertise) is necessary. Next to these concerns we would like to address the concerns as stated in the reaction of EUHA (see the attachment for a summary of these concerns). Erasmus MC is one of the EUHA members and we like to emphasize the importance of collaboration with EUHA on future actions related to this consultation. Response to European Health Emergency Response Authority

European Health Emergency Preparedness and Response Authority (HERA) should have two main focus areas: pandemics and disaster preparedness. In addition, particular attention should be given to the important links between pandemic and disaster preparedness. Both in methodologies, as in cascading effects. The HERA should also be acting as a hub, primarily aiming at pandemic and disaster research progress and added at value creation: it should serve a clear global health, economic and social purpose. The Authority should operate a so-called hubs-and-spokes model, in which the HERA hub runs a focused package of research and innovation activities addressing key challenges, with spokes in the various member states contributing with a defined set of activities based on their specific strengths. The common goal supported by funding is to ensure further advancement in pandemic and disaster preparedness. HERA should develop pandemic and disaster preparedness – driven life sciences, social sciences, health and economic developments and facilitates further elaboration and social embedding of pandemic and disaster preparedness. The HERA should have from start an European - wide and international focus: implementing European and internationally oriented activities, programs and projects. The HERA should operate as a hub for foundational research for key challenges in global preparedness and response, and create with the spokes in the various member states a testing ground for potential solutions, and is committed to come up with purposeful contributions to the problems the world faces. The HERA will act as a network for knowledge and training to create the next generation scientist s and practitioners in the field of disaster and pandemic preparedness.

Response to Europe’s Beating Cancer Plan

Erasmus University Medical Center (Erasmus MC), Rotterdam, the Netherlands congratulates the European Commission on its initiative to launch the Europe’s Beating Cancer Plan. As a large scale University Medical Centre, with a dedicated Cancer Institute, Erasmus MC applauds the EU’s initiative to fight cancer and fully supports the EU’s commitment to reduce cancer mortality by 25% in 2025. Erasmus MC is committed to achieving a healthy population and pursuing excellence in healthcare through research and teaching. Within our Erasmus MC Cancer Institute, in 2018, our 143 medical specialists see many cancer patients on a yearly basis, with almost 47 thousand unique patients. Our mission is to reduce the number of people developing cancer, to have more people survive cancer and to improve quality of life for cancer patients. In cancer research and care, we work to provide optimal, tailored healthcare (precision medicine), to promote health (prevention), to educate professionals, and to further understand, predict and prevent cancer through fundamental, translational and clinical research. Erasmus MC fully supports the need to further identify the modifiable lifestyle factors (incl. diet, vaccination) and environmental risk factors that contribute to cancer development in a heterogeneous population, to increase citizen-awareness of those risk factors, and to expand the scope of risk-based screening. It is important that no cancers – rare, complex and/or common – are excluded from the focus of Europe’s Beating Cancer Plan. That all possible interventions, from lifestyle to surgical to pharmacological, are explored to get the best possible results for the citizens of the European Union and beyond. European networks sharing best practices and expertise will play an important role in beating cancer throughout the Union. Response to Multiannual Financial Framework: Erasmus Programme 2021-2027

Erasmus University Medical Center Rotterdam (Erasmus MC) congratulates the European Commission with the Erasmus proposal, and specifically commends the effort made to design Erasmus by “change through evolution rather than revolution” through building on the solid base created in Erasmus+. As a large university medical center, Erasmus MC fully endorses concept of lifelong learning, underpinning the various sub programmes. Moreover, Erasmus MC welcomes the proposed changes to the programme, especially the focus on digital skills and forward-looking fields; and a significant simplification for the international actions. Finally, Erasmus MC applauds the aim to raise awareness on the important role of sports in promoting physical activity and a healthy lifestyle. Erasmus MC endorses the currently proposed budget of 30 billion euro in order to address all goals set for Erasmus. Furthermore, Erasmus MC would like to offer the following recommendation to the European Commission concerning the synergy with Horizon Europe: Erasmus MC recommends to prioritize synergies between research and innovation and education activities particularly in Partnerships for Innovation. Erasmus MC is most willing to contribute to the further preparation of the Erasmus programme.

Response to MFF: 9th Framework Programme for Research and Innovation and Rules for Participation and Dissemination

Erasmus University Medical Center Rotterdam (Erasmus MC) congratulates the European Commission with the Horizon Europe proposal, and specifically commends the effort made to design Horizon Europe by “change through evolution rather than revolution” through building on the solid base created in Horizon 2020. As a large university medical center, Erasmus MC fully endorses the thematic programming for the Health cluster in Pillar 2. ‘Health throughout the life course’ in particular is an important Area of Intervention to address the challenge of the growing burden of chronic diseases in Europe, with the start of life offering a great - and still to be fully explored – window of opportunity for intervention. Erasmus MC recommends to increase the currently proposed budget to 120 billion euro in order to address all goals set for Horizon Europe. Furthermore, Erasmus MC would like to offer the following recommendations to the European Commission for the further development of Horizon Europe: Pillar 1: - Excellence should remain the sole evaluation criterion for the ERC. - The proposed budget for MSCA should be doubled. MSCA is a very popular programme among European scientists as reflected by a high degree of oversubscription and a low funding rate even for excellent proposals. Furthermore, it is an important promoter of career development of the next generation of European scientists, and essential for the streamlining and harmonization of European training in research and innovation. Pillar 2: - Considering the increasing health challenges that Europe is facing, the current budget for the Health cluster is too limited and should be increased. An adequate budget needed to meet the health ambitions set for Horizon Europe should be defined. - Collaborative fundamental research should have a clear position in the Health cluster in order to tackle knowledge gaps that currently prevent Europe from making significant advancements in addressing health challenges and bringing new knowledge further towards innovation with impact for society. Pillar 3: - Erasmus MC endorses the substantial financial support for innovation and welcomes the EIC. It is important that the positioning of EIC and EIT in Horizon Europe is transparent, effective and without overlap. Further simplification: - The wide variety of instruments in Horizon 2020 creates a high level of complexity and often unclarities for applicants. A limited number of flexible instruments in Horizon Europe may better deliver the expected impact of the various calls and topics in a more effective way. Erasmus MC welcomes the future release of further details on the definition of the missions, and is most willing to contribute to the further preparation of the Horizon Europe programme.
