The non-profit purposes of international utility of the Association shall be, within the European Union and worldwide to: (a) Provide an international powerful and cohesive platform in which diverse stakeholders are working together to further the improvement of diabetes prevention and diabetes care; (b) Enable healthcare systems to cope with the diabetes pandemic while achieving the best possible outcomes for people with diabetes; (c) Ensure the translation of research into policy actions towards better diabetes care at national and international level; and (d) Defend, represent, and promote in the broadest sense of the word the common interests of its Members.

Response to EU cardiovascular health plan

Diabetes, obesity and cardiovascular diseases (CVD) are closely interconnected. Therefore, the upcoming Cardiovascular (CV) Health Plan should address the triangle of diabetes, obesity and CVD. The effective prevention, detection and management of all types of diabetes is key to preventing CVD. Type 2 diabetes (T2D), obesity and CVD are recurrent conditions that share risk factors and underlying biological mechanisms. T2D is a major cause and risk factor for CVD and people with T2D have a CV risk 2-3 times higher than people without diabetes and see their life expectancy reduced by 10-14 years. People with type 1 diabetes (T1D), a chronic, autoimmune disease have mortality rates 318 times higher than the general population, largely due to CVD-related complications. The cost of diabetes in Europe reached 176 billion in 2021, with 75% spent on treating preventable complications such as CVD. Healthcare systems remain oriented toward acute care, with models and budgets siloed by individual conditions, neglecting the overlapping nature of chronic diseases. This results in fragmented care, inefficiencies, and missed opportunities for prevention, timely detection and early intervention. The CV Health Plan offers a unique opportunity for a fundamental shift toward proactive, long-term, person-centred, and holistic care models for chronic conditions. Integrated strategies should address blood glucose, blood pressure, cholesterol, and weight not only to improve diabetes outcomes but also to lower the burden of heart disease, stroke, and obesity - creating a broader impact on public health. Key asks from the diabetes community for the CV Health Plan: 1. Early Detection to enable timely treatment, prevention of complications, improved quality of life, and better health system efficiency: a. Adopt Council Recommendations for targeted, integrated health checks including screening for all types of diabetes. b. Create a platform for primary health care and community health workers to strengthen primary and community care and support the implementation of cardiometabolic health (CMH) checks. c. Adopt EU-specific targets for diabetes prevention, detection, coverage and care. d. Adopt standardised guidance through European Guidelines and Quality Assurance schemes on screening, diagnosis, treatment and follow-up. 2. Equitable Care to reduce disparities in CMH related care across the EU: a. Establish CMH centers of excellence on integrated care. b. Establish a Knowledge Center on CMH, a pan-European CMD data system capable of providing live status reports on CMD, care quality and clinical outcomes. c. Develop a comprehensive CMH training programme, to establish integrated CMH curricula and certifications to train HCPs across Europe. d. Support Member States in transitioning their healthcare systems toward an integrated CMH approach, through the European Semester. 3. Empowering people to manage the condition and thrive as active citizens: a. Set up a CMH Policy Platform, to place people living with CMD at the centre of research, regulatory, policy and evaluation processes. b. Set up a Subgroup on CMH as part of the Commissions Expert Group on Public Health. c. Launch a project on Health Literacy for CMH Prevention and Care. 4. Embracing Science and Technology to facilitate data collection and reduce risk of complications: a. Establish a pan-European CMH Registry to enhance data collection. b. Fund CMH research under EU research and health programs. c. Scale up and interconnect digital and data infrastructures to support real-world monitoring of CV outcomes in people with diabetes. d. Develop and fund dedicated clinical research initiatives to generate robust, T1D specific evidence on CV risk management. e. Promote personalized CMD management through digital health innovations. f. Ensure research and policy frameworks are inclusive of underserved populations, including children and adolescents with T1D and individuals with multimorbid T2D.
