Response to Europe’s Beating Cancer Plan

The Societal Impact of Pain (SIP) (www.sip-platform.eu/sip-joint-statement) welcomes the European Commission Roadmap on Europe’s Beating Cancer Plan. SIP believes that as pain features heavily in the lives of cancer patients and survivors it must be addressed in the plan. Estimated pain prevalence in cancer patients is around 70%; 80% for those with advanced stages. Most cancer pain is caused by a tumor but can be related to treatment. In cancer survivors (curative treatment completed), between 33% and 40% suffer from chronic pain that interferes with functioning. In this context, pain management in cancer patients can reduce workplace absenteeism and improve quality of life. In palliative care, pain management can improve patients’ quality of life. Cancer patients face discrepancies in pain management and palliative care. SIP welcomes the instruments to improve the “quality of life for cancer patients, survivors and carers” noted in the Roadmap, including platforms and resources to support the dissemination of best practices for pain management. Based on the 2016 SIP policy recommendations on the relevance of pain in cancer care and rehabilitation (cf. attachment), SIP invites the European Commission to consider the following areas for intervention with potential instruments and measurable targets: Continuous education - through appropriate trainings and awareness programmes, ensure that all those involved in patient cancer care are educated in pain assessment and management, and other principles of palliative care. Such programmes should actively include patients. Possible instruments: the EU should support pilot training projects and promote mobility of healthcare professionals to exchange best practices. It should encourage access to national pain education. Pain research – EU and national pain research (including other causes of distress in cancer and palliative care patients), using routinely collected data on pain, the newly adopted ICD-11 classification, updated national cancer registries and digital health solutions. Possible instruments: the EU and Member States should allocate research funding, support data collection pilot projects using digital solutions, and promote the exchange of best practices in the establishment/updating of cancer registries. Allocation of resources – national cancer strategies (including workforce planning) to ensure the sustainability of cancer care services/treatment, and the availability of cancer services and patient pathways which include pain assessment and management. Possible instruments: the EU should encourage Member States to allocate resources for pain management and palliative care. SIP is a unique example of multi-stakeholder partnership bringing together patients, citizens, healthcare professional organisations and industry to work towards the improvement of pain care. It is frequently recognised as a strategic partner in pain policy-making, e.g.: Reference to pain in the EU position ahead of the 2011 UN high-level meeting on the prevention and control of non-communicable diseases. Provision of the EU Directive on the application of patients’ rights in cross-border healthcare foreseeing that an objective assessment of ‘the degree of the patient’s pain’ must be used to assess the right to cross-border healthcare. 2014 informal Conclusions of the EU Health Ministers, promoting the use of palliative care and pain therapy across Member States. 2017 Maltese Council Conclusions on ‘Encouraging Member States-driven Voluntary Cooperation between Health Systems’ calling for assessment of the level of access to treatment for people with chronic pain. Recognition by the European Commission of SIP as a Stakeholder Group, a long-term platform to collect best practices and provide a forum for dialogue on pain policy. After being selected as a 2018 European Commission Thematic Network, SIP produced a joint statement on the societal impact of pain to build on existing EU health policies.

Response to European Electronic Health Record (EHR) Exchange Format

DIGITALEUROPE strongly supports the Roadmap and urges speed and urgency in delivering the ability for summaries of health records and prescriptions to be exchanged over borders. The ultimate goal is to improve delivery of care, better health outcomes for patients and increased efficiencies of healthcare systems across Europe. We encourage interoperable exchange formats to be achieved. But we also believe there is an opportunity to begin a more ambitious project to deliver complete health record files to be shared across the EU with access and control given to the individual European Citizens. The EU is a global leader in defining data protection rights and protections and should exploit this to accelerate the flow of data in the EU. One area to accelerate an EU citizen’s access to their data would be the exchange of data through the internet, Apps and to devices. For example Representational State Transfer (REST) is an architectural style that defines a set of constraints to be used for creating web services. In a RESTful web service, requests made to a resource's URI will elicit a response with a payload formatted in either HTML, XML, JSON, or some other format. One standard that takes advantage of a wide range of such standards and architectures is the FHIR (Fast Healthcare Interoperability Resources) specification, which is a standard for exchanging healthcare information electronically. The EU has an excellent opportunity to accelerate the development of eHealth solutions and data that are interoperable across borders and across interfaces and devices.
