HIV Outcomes is a pan-European, collaborative initiative uniting community and patient advocates, healthcare professionals, academics, and the industry, aiming to improve the health-related quality of life (HRQoL) for all people living with HIV throughout their life span. We welcome the launch of a new EU Gender Equality Strategy as an opportunity to advance inclusive policies supporting dignity, rights, and health across Europe. Given HIV Outcomes focus, our feedback centres on advancing the rights and wellbeing of people living with HIV, through a gender-sensitive and intersectional lens. Intersectionality between HIV and gender equality: We commend the inclusion of highest standards of health as a core principle of the Womens Rights Roadmap and support its integration into the upcoming Strategy. Healthcare systems often fail to reflect the needs and lived experiences of women, both cis-gender and trans, and gender-diverse people. These groups face additional barriers to care due to stigma, sexism, and other forms of marginalisation that impact their HRQoL and wellbeing. For example, in the EU/EEA, migrant women are disproportionately affected by HIV and face further obstacles linked to legal status, xenophobia, racism, and the criminalisation of HIV. These intersecting vulnerabilities limit access to quality of care and have serious impact on their health outcomes. Therefore, we call for an intersectional approach that fully integrates HIV and other Sexually Transmitted Infections (STIs) into the Gender Equality Strategy, recognising intersecting vulnerabilities and health inequalities. Promoting HRQoL: The Strategy should go beyond clinical outcomes to include indicators that reflect mental health, wellbeing, and social inclusioncore elements of the WHO definition of health. We recommend the inclusion of HRQoL indicators in national and European monitoring systems that capture gender-specific experiences of people living with HIV, including stigma, discrimination, and unmet support needs, to inform both person-centred care and evidence-based decision-making. Long-term care and ageing with HIV: Women and gender-diverse people face persistent disparities in access to respectful, inclusive care, which widen with age and the growing number of comorbidities. Gender-sensitive screening and preventative interventions are essential for equal outcomes, including guidance on self-management. Throughout their lives, women also need access to informed family planning, pregnancy care, and menopause support, services often fragmented and not tailored to those living with HIV. The Strategy should promote training on HIV and gender inclusivity, ensure stigma-free long-term care, and develop integrated care pathways that address ageing and gender-specific needs. Gender-based violence: Gender-based violence, compounded by HIV and gender-related stigma, undermines access to care and support. The Strategy must prioritise robust anti-discrimination measures in healthcare and social services, while addressing structural inequalities that perpetuate violence. EU action should include training for healthcare and social workers to foster dignity and equitable outcomes. Data collection and research: Addressing the gender data gap is essential to achieving equitable health outcomes, with women living with HIV remain underrepresented in research, policy, and programmes. The Strategy should prioritise collecting disaggregated data on women and gender-diverse people living with HIV, particularly via the ECDC. Investment in inclusive, gender-sensitive research and programmes, especially on HRQoL in underrepresented groups such as trans people, migrants, and older adultsis key to improving access to care and wellbeing.

FIPRA is submitting this input on behalf of the Smart Food Technologies (SFT) Coalition. The Coalition for Smart Food Technologies is an initiative founded in September 2023 and sponsored by HelloFresh, Keep-It Technologies, and Zebra Technologies. The SFT Coalition brings together actors across the food value chain with the aim of promoting policies and reforms that encourage innovation and enable the use of new shelf-life technologies to prevent food waste and enhance food safety. The SFT Coalition welcomes the revision of the Waste Framework Directive on Food and Textiles. The ambition of reducing food waste is urgently needed and of high importance in a global context of food insecurity and climate change. Whilst the SFT Coalition finds the implementation of food waste reduction targets as a first step in the right direction, it also believes that the European Commissions current proposal does not take on a fully prescriptive approach to address the low-hanging fruits of food-waste reduction. For instance, Member States could meet a third of their food waste reduction targets in retail, food service, hospitality, and at home by addressing the problems surrounding food waste linked to static date markings which are responsible for 10% of food waste across the Union. Solutions to the flaws of static date markings could be achieved through the implementation and use of smart food technologies.These technologies could offer a new standard for food labelling when used systematically across the value chain, providing customers and retailers more precise and accurate information on food freshness. The SFT Coalition encourages legislators to adopt a more ambitious approach to this proposal by ensuring that smart food technologies, such as temperature indicators or sensors, contribute actively to Member States food waste prevention measures, especially by introducing their alternative use instead of traditional static date-marking. Response to Proposal for a Council Recommendation on long-term care

HIV Outcomes, a network comprising of over 35 individuals and organisations advocating for the well-being of people living with HIV, welcomes the European Care Strategy and its foreseen reforms in long-term care (LTC), and is pleased that improving well-being for those in care is one of the Strategy’s objectives, as noted in the ‘2021 LTC in EU’ report (hereinafter the report). In line with our input to Green Paper on Ageing, this Strategy should improve well-being for people in LTC and living with a long-term condition. According to the European Health Interview Survey, people living with long-term conditions self-report poorer well-being than the general population and often face greater LTC dependency. As highlighted in the Green Paper and the Report on ‘The Impact of Demographic Change’, the growing burden of long-term conditions associated with the increase in life expectancy, will become a major challenge for care systems, so the well-being of people in care living with such conditions should be a key target of this Strategy. It is known how complex it is to improve well-being for those in care living with a long-term condition. We call for the adoption of integrated models of LTC based on multidisciplinary and cross-sectoral cooperation between care and healthcare practitioners, as they are best positioned to meet the needs and improve well-being for this population. We concur with the report in that LTC and healthcare policy should be seen together because not only does healthcare policy have a direct impact on LTC, but also greater dependency of LTC and healthcare services is reported among those living with long-term conditions. The provision of integrated LTC is thus essential and should be encouraged in this Strategy. In moving towards integrated models of LTC, we want to raise awareness of the importance of health-related quality of life (HRQoL) – an indicator to capture the multi-dimensional issues challenging well-being for those living with a long-term condition. Traditionally, HRQoL is used in health technology assessment but is gaining prominence in oncology and HIV as a guiding paradigm towards more targeted and person-centred models of care. As such, it can help assess the quality of LTC by offering a more precise and measurable indicator. People living with HIV are affected by these issues and should be targeted in the Strategy, for HIV has turned into a long-term condition thanks to recent therapy breakthroughs, yet they report poorer well-being than the general population. They demand quality LTC to manage the needs associated with the condition and its often disabling co-morbidities, such as mental disorders, osteoporosis, respiratory diseases, etc. The impact of these co-morborbidities on well-being may be exacerbated by discrimination and stigma, and by the comorbidities linked to ageing. In care settings, it has been reported episodes of stigma and discrimination towards people living with HIV and/or other stigmatised conditions. Protecting those vulnerable from abuse or neglect is rightly reflected in the report as a prerequisite for quality LTC. The Strategy should, therefore, include actions to tackle stigma and discrimination and educate care workers. In addition, with the increase in life expectancy and the breakthroughs in retroviral therapy, the number of old people living with HIV is larger than ever and can be subject to issues of triple stigma related to ageism, HIV and anti-LGTBQ prejudice. In view of this evidence, we hence recommend that this Strategy: 1. calls for the adoption of integrated models of LTC as best positioned to meet the needs of those in LTC and living with long-term conditions, 2. recognises HRQoL as a valuable indicator to guide policy reforms towards more targeted and person-centred models of LTC, 3. addresses stigma and discrimination in care settings (e.g. towards people living with HIV and other stigmatised conditions).

Response to Cancer Screening Recommendation

The ACHIEVE Coalition, advocating to end hepatitis by 2030 and comprising patient associations, academics, clinicians and industry, welcomes an update to the 2003 Council Recommendation on cancer screening and recommends the inclusion of liver cancer and its key determinant viral hepatitis in the updated Recommendation since liver cancer is largely preventable and treatable when diagnosed early. Liver cancer has become a major health burden in the European Union and the 10th most frequent cancer among men, with similar incidence rates than stomach and pancreas and higher prevalence than leukemia, brain, larynx or testis cancer. Overall, Europe has the second-largest share of liver cancer cases and deaths in the world (10%) well above North (4.7%) and Latin America (6.8%) and only surpassed by Asia (67%). Each year, there are up to 90,000 new cases and around 80,000 deaths in Europe, likely amongst the most vulnerable groups in our societies. By 2040, it is estimated that new deaths and cases of liver cancer will exceed 100,000 per year. The EU has seen a dramatic increase in liver cancer mortality and morbidity due to insufficient progress against viral hepatitis, excessive alcohol consumption and a rise in obesity rates. Unlike other solid cancers, which used to have a high fatal burden but for which incidence and mortality rates are stable or declining, in the EU there has been a 70% increase in liver cancer-related deaths in the past three decades, highlighting the need for coordinated preventive action across Europe. Altogether, liver cancer today has the lowest survival rate among all cancers monitored by the Joint Research Centre. The burden of liver disease can be significantly reduced with the preventive tools now available. This Recommendation should therefore facilitate the roll-out of screening programmes for liver cancer targeted at people with jeopardized liver health and help expand efforts against viral hepatitis. Viral hepatitis B (HBV) C (HCV) are responsible for over 70% of liver cancer cases and are defined as cancer preventable risk factors by WHO and recognised as such in Europe’s Beating Cancer Plan. HBV is preventable with a vaccine and, when there is a chronic infection, the disease can be controlled. For its part, chronic infection with HCV is curable with effective treatment. In addition, there is a significantly elevated cancer risk for people infected with chronic hepatitis D (HDV) which is a co-infection of HBV, estimated to be present in 3% of people living with HBV in WHO Europe (13 million). Worryingly, up to 90% of people infected with hepatitis in the European Union are unaware of their infection (9-10 million), especially vulnerable groups who are disproportionally affected by HCV such as people who use drugs. Without being linked to care, their liver health gradually deteriorates and can eventually lead to cancer. Health economic models suggest that the elimination of viral hepatitis C alone would reduce the burden of liver cancer through morbidity and death by more than 65% In view of this evidence, we hence recommend that in the updated Council Recommendation: 1. Liver cancer be included in the list of cancers which are specifically mentioned, alongside breast, colorectal, cervical and other cancers, and viral hepatitis be mentioned as a key determinant for liver cancer (currently Recital 3); 2. Viral hepatitis be included in the list of criteria when taking account of the specific needs for persons who might be at a higher cancer risk (currently Recital 11); 3. Monitoring and screening for liver cancer among specific risk groups whose liver health is and/or can be jeopardized (e.g., through a chronic infection with viral hepatitis) be included in the measures, as the efficacy of such preventive measures has been proven. The EU can play a key role in supporting Member States to reduce the burden of liver cancer. ACHIEVE reiterates its support for this proposal.
